COLONIAL SCHOOL DISTRICT NON-PUBLIC PROOF OF RESIDENCE TRANSPORTATION REQUEST FORM 
 
Dear Parent or Guardian:
 
The Colonial School District provides transportation for Non-Public School children pursuant to Pennsylvania State Law. Transportation will be provided to and from an accredited Non-Public School that is located no more than (10) miles from the Colonial School District boundaries. If you believe you are eligible for transportation and desire to have it provided for the upcoming school term, please complete this form. 
 
Families that are leasing a property within the school district must fill out the "Non-Public Lessee Residency Affidavit." If you are living with another family you must fill out the "Non-Public Transport Multiple Occupancy." The transportation department will verify property owners via the Montgomery County Property records website. The District will verify residency from time to time, and falsification of any information may lead to prosecution and or fines.
 
Please note if your child is a current Public School student transferring to a Non-Public School you must first withdraw from the Public School before transportation can be set up.  Please contact the school of attendance to withdraw your child.
 
Your cooperation in providing complete answers and all information is requested and will help us to provide the best possible transportation service to our Colonial residents. 
 
At the start of the school year, late registrations may wait three to four weeks for transportation if paperwork is not in our office by July 1.
 
Complete a separate form for each child eligible for transportation for the school year.
